Collection of Information and Limits of Confidentiality:
The law protects the privacy of information that I collect about you or your child. According to Health Regulations 1996, I must keep the information I collect for 10 years. As per my ethical code, information gathered by me will be kept confidential and not released to anyone without your consent. Information regarding you or your child will kept securely in a locked office cabinet. In some cases, your child’s daily progress data will be stored securely on-site (e.g., at school).
However, there are some situations in which I could be obligated to take actions in order to protect you, your child, or others. This could occur if there was high probability of physical harm to you, your child, other individuals, or society. This may require that some information about you or your child be revealed, but only to persons who require it to ensure the safety, and care of those people affected. If such a situation arises, every effort will be made to fully discuss this matter before taking any action, and disclosure will be limited to what is necessary. Your child may benefit from services provided from a number of health providers (e.g., Speech Language Therapy, Social Worker). We may need to share information between each other in order to provide consistent and effective services. I will inform you when this needs to occur and what information will be discussed. These other professionals are obligated to adhere to the rules of confidentiality as mandated by the Health and Disability Commissioner Act (1994) and their professional ethical requirements.
When I provide services for your child via another agency, I am generally required to communicate with that agency about my services and your child’s progress (e.g., Social Worker, other psychologists).
Termination of services: You have the right to withdraw consent at any time.
